It would have to be done at the X resource level, mostly on a per
application basis. This works for XTerm:

XTerm*VT100.translations:#override\n\
        <Key>Insert: insert-selection(PRIMARY)

I don't know how far you would get if you tried:

*translations:#override\n\
        <Key>Insert: insert-selection(PRIMARY)
